Job summary

Harborview Medical Center, part of UW Medicine, seeks a Social Worker for inpatient care management. This per diem, part-time role provides direct social work services on medical/surgical units and engages in teaching activities.

The incumbent will exercise independent judgment with a multidisciplinary team to support safe discharge planning and complex care coordination. Required is a Master of Social Work (CSWE-accredited) and at least one year of health-care social work experience.

Qualifications

• Master of Social Work from a CSWE-accredited program.
• At least one year of full-time social work experience in a health care setting.
• Experience can include practicum or employment in related roles.

Responsibilities

• Provide specialty psychosocial consultation to health care teams.
• Offer psychosocial support to patients and families.
• Collaborate with the multidisciplinary team to develop safe discharge plans.
• Coordinate care for patients with complex social/medical histories.
• Provide case management and counseling services in specialized programs.
• Make timely referrals to hospital and community agencies per reporting laws.
• Perform related duties as assigned.
