Job summary

Harborview Medical Center seeks a Social Worker for inpatient care management and bed readiness coordination. You will support timely discharges, work with a multidisciplinary team, and help medically ready patients transition to appropriate post-acute settings.

This per diem, day-shift role focuses on Medicaid/Medicaid-like programs for efficient discharge planning and reducing barriers. Strong inpatient social work experience and knowledge of transition processes are preferred.

Qualifications

• MSW from a CSWE-accredited program.
• At least 1 year of full-time social work experience in health care.
• Experience with discharge planning and care coordination.

Responsibilities

• Provide specialty consultation to staff on psychosocial aspects of care.
• Support ECM referrals and coordination with care management staff.
• Offer psychosocial support to facilitate discharge from inpatient to post-acute settings.
• Coordinate post-acute follow-up with interdisciplinary team for ongoing needs and discharge planning.
• Work across settings including inpatient, SNFs, community residences, and shelters.
