Job summary

Mayo Clinic in Phoenix seeks a Registered Nurse Case Manager to coordinate patient care across the continuum within an interdisciplinary team. You will ensure appropriate utilization management, care coordination, and timely documentation to deliver high-quality, cost-effective care.

The role includes leadership in case management education and involvement in committees and research. The position requires Arizona RN licensure with BLS, 3 years acute nursing preferred and 1 year Case Management

Qualifications

• Arizona RN license and current licensure requirements.
• One year RN experience in an applicable care setting if not ACEN/CCNE/NLN-CNEA accredited.
• RN Case Management experience or completion of Mayo Clinic fellowship within six months.

Responsibilities

• Collaborate with an interdisciplinary team to coordinate patient care plans.
• Lead education on case management concepts to improve outcomes and costs.
• Ensure utilization management and timely clinical documentation across transitions.
• Communicate with providers, patients, and families to advocate for patients.
• Bridge communication between care teams and external agencies for quality care.
