Job summary

Augusta Health is seeking an Inpatient RN Case Manager in Fishersville, VA. The role coordinates patient care with the interdisciplinary team to develop safe discharge plans aligned with patients' goals of care and transitions them to the next level of care in a timely, compassionate manner.

The ideal candidate holds a BSN, Virginia RN license (or compact), and at least 2 years in an acute hospital setting.

Qualifications

• Bachelor's degree in nursing or equivalent experience.
• Registered nurse license in the Commonwealth of Virginia or a compact state.
• Minimum of 2 years in an acute hospital setting.

Responsibilities

• Coordinate patient care with the interdisciplinary team to plan post-hospital needs.
• Assess and evaluate discharge plans aligned with patient/family goals of care.
• Ensure timely and safe transitions to the next level of care.
• Maintain effective inter-professional collaboration across clinical teams.
