Piedmont Augusta Hospital is seeking a Care Manager to coordinate care, oversee progression of care, and drive proactive discharge planning for a caseload of patients.

Responsibilities include expediting timely, safe discharges and collaborating with nurses and physicians to optimize patient outcomes in an acute/post-acute setting.

RN licensure in Georgia is required; a Master's degree is preferred and prior experience with InterQual or Milliman is valued.

Formación

• Graduate of Nursing Program required.
• Master's degree preferred.
• RN licensure in Georgia required.
• Case management certification preferred.
