Job summary

American Traveler, LLC. is seeking an RN Case Manager with inpatient case management experience for a Day Shift role in California. You will manage complex discharge planning and coordinate care from pre-admission through post-discharge in RevCycle - Care Management.

The position requires a CA RN license, BLS, and 2+ years in specialty with recent practice. Inpatient experience and strike force exposure for KP or HCA CA facilities are a plus.

Qualifications

• Active CA RN license required.
• BLS certification from an approved vendor required.
• BSN preferred.
• Prior care management experience preferred.
• Case management certification preferred.
• Inpatient case management experience preferred, ideally with strike force experience for KP or HCA CA facilities.
• Minimum 2 years of experience in specialty required, with recent experience within the last 3 months.

Responsibilities

• Unit: RevCycle - Care Management
• Day Shift, 8:00 AM-4:30 PM, Tuesday-Saturday, 40 hrs/week
• No on-call requirements
• Manages complex discharge planning for a defined patient population
• Focuses on complex patients, frequent ED utilizers, chronic pain patients, substance abuse patients and homeless patients
• Coordinates continuum of care for patients from pre-admission through post-discharge
• Acts as liaison between patient, family, nurse, physicians, multidisciplinary team, and payer
• Collaborates with acute care case managers and coordinates transfers to tertiary centers
• Monitors appropriateness and timeliness of care, resource utilization, and discharge planning
• Provides oversight and data collection for regulatory and accreditation compliance
• Conducts focus study reviews for frequent emergency department visitors
• Candidates with more than 5 days of requested time off will not be considered
• Candidates with a prior hit on their license will not be considered
• Accepts local candidates at the same pay rate as travel candidates
