Ascension Health in Indianapolis, IN is seeking an experienced RN for inpatient case management to lead care from admission through discharge. The role focuses on seamless transitions, advocacy, and resource coordination to support recovery and safe discharge home or to the next level of care.

The position requires an active RN license, nursing diploma, and acute care case management experience, with CCMC preferred. Competitive benefits accompany a Monday–Friday schedule.

Qualifications

• Registered Nurse required prior to hire date or transfer date.
• CCMC certification preferred; ACM accepted.
• Diploma from an accredited school/college of nursing or equivalent licensure.
• Acute care Case Management experience.
• At least 2 years of acute care inpatient nursing.
• Post Acute Care Coordination experience is beneficial.

Responsibilities

• Lead the interdisciplinary care strategy from admission to discharge to ensure a seamless, safe transition home or to the next level of care.
• Advocate for patient needs by removing financial or logistical barriers and ensuring necessary resources.
• Evaluate medical necessity and care levels to optimize hospital and insurance resources.
• Collaborate with families, clinical teams, and insurers to build a unified post-discharge plan.
