Job summary

Baptist Health Floyd in Southern Indiana is seeking an experienced RN Case Manager. The role is full-time, Monday to Friday, 8:00a-4:30p with occasional weekend/holiday rotation, located in the Southern Indiana/Louisville area.

The position leads assessment, planning, facilitation, and advocacy for patient options and services to ensure quality, cost-effective outcomes. The case manager collaborates with a care coordination team to empower patients to access high-quality, safe, and efficient

Qualifications

• Registered Nurse with graduation from an accredited school.
• Current licensure in the State of Indiana.
• Bachelor's degree preferred.
• Minimum of 3 years' experience in acute care nursing or Home Health Care.
• 3-5 years of Discharge planning/Case Management/Utilization Review experience preferred.

Responsibilities

• Lead the collaborative process of assessment, planning, facilitation, and advocacy for options and services to meet patient health needs through communication and use of available resources.
• Promote quality cost-effective outcomes including access to care and appropriate utilization of resources.
• Empower people to understand and access quality, safe, and efficient health care services.
