Qualifications

Responsibilities

• Assess complex discharge needs and plan care with multidisciplinary team.
• Lead daily multidisciplinary rounds to identify and address barriers to care.
• Educate patient and family about options and resources for safe discharge.
• Coordinate care with physicians, staff, payers and community resources.
