Qualificações

• Graduate of an accredited school of nursing.
• Current state RN license required.
• Current BLS certification.
• Thorough knowledge of clinical nursing skills.

Responsabilidades

• Coordinate and implement case management activities within the hospital.
• Lead and participate in multidisciplinary care conferences and rounds.
• Plan and coordinate all necessary care services for discharge and transitions of care.
• Act as liaison for patient/family/legal representative throughout hospitalization and transitions.
