Job summary

Qualifications

• BSW preferred and 2+ years of medical/hospital experience.
• At least 2 years of case management experience preferred.
• Must comply with licensing rules and Benefis Health System policies.

Responsibilities

• Coordinate and implement case management activities for the patient during hospitalization.
• Remain the point of contact for patient/family/legal representative until transition.
• Review care and treatment for appropriateness against criteria for referrals.
• Monitor and adjust referrals and resource requests as needed.
• Send referrals to appropriate service lines for review.
• Attend daily care rounds and update the team on changes in patient condition.
• Collaborate with multidisciplinary care team and patient navigators.
