Job summary

Providence Health and Services is hiring a Care Manager for our Everett facility. This RN role supports at‑risk patients in an acute care setting with a focus on assessment, planning and coordination of care.

The position requires a Washington RN license and an associates degree in nursing, with opportunities for growth within a respected health system. The Care Manager will collaborate with care teams, manage discharge planning, and advocate for patients.

Qualifications

• WA RN license is required.
• Associates degree in nursing required; bachelor’s degree preferred.
• Minimum 2 years acute care experience in Medical‑Surgical or Inpatient settings.
• 1 year experience in care management or utilization review or related program.

Responsibilities

• Perform assessment and planning of patient care.
• Coordinate care across inpatient teams and services.
• Manage resource utilization and discharge planning.
• Document interventions and ensure regulatory compliance.
• Advocate for patients and families throughout care journey.
