Qualifications

• 2 years of acute care experience in a Medical Surgical or Inpatient setting.
• 1 year in care management or completion of TIPS/Case Management Orientation program.

Responsibilities

• Assess and plan patient care; develop care plans.
• Coordinate care across teams and settings.
• Manage resource utilization and discharge planning.
• Document interventions and ensure regulatory compliance.
• Advocate for patients and families throughout the continuum.
