Qualifications

• Associate's Degree in Nursing required.
• Washington RN license required.
• 2 years of Acute care experience in Medical Surgical or Inpatient setting.
• 1 year of care management/ utilization review experience or completion of TIPS/Case Management Orientation Program.

Responsibilities

• Assess and plan patient care and coordinate services.
• Coordinate care across the flow of treatment and discharge planning.
• Document interventions and ensure regulatory compliance.
• Advocate for patients and ensure appropriate resource utilization.
