Job summary

Murfreesboro Rehabilitation Hospital is seeking a PRN Rehab Inpatient Social Worker to join a fast-paced, multidisciplinary team. The role focuses on psychosocial assessment, discharge planning, and helping patients with recovery from injury or illness in an inpatient setting.

The position requires a Bachelor's degree in Social Work and licensure eligibility; CPR and de-escalation certification are required.

Qualifications

• Bachelor's degree in Social Work from an accredited college or university.
• One or more years’ experience working with patient population preferred.
• Eligible for state licensure as LSW/LAC/CAC/RAC as required by the state.
• CPR and de-escalation/restraint certification required.
• First aid may be required.

Responsibilities

• Conduct social history assessment and develop treatment plans.
• Coordinate programs to meet social and emotional needs.
• Promote recovery and highest level of social functioning.
• Provide crisis management and family sessions.
• Participate in multidisciplinary treatment team meetings.
• Manage caseload and patient chart documentation.
• Facilitate education, process groups, and referrals.
• Maintain professional attitude and address patient concerns.
