Job summary

• Coordinate individualized care plans with therapy disciplines and physicians.
• Administer medications, treatments, and interventions while monitoring effectiveness.
• Maintain accurate documentation of assessments, interventions, and education in the EHR.
• Educate patients and families on self-care strategies and discharge instructions.
• Collaborate with interdisciplinary team to enhance patient engagement and safety.
