Job summary

LiveWell Homecare Agency in Austell, GA is seeking an Occupational Therapist to assess, plan, and implement rehabilitative programs that improve daily activities, mobility, cognition, strength, and range of motion for patients in home care settings.

You will collaborate with physicians, families, and staff, document evaluations and treatment plans, and uphold ethical standards, confidentiality, and AIDET-based customer service while promoting quality care and outcomes.

Qualifications

Bachelor’s or Master’s degree in Occupational Therapy.
OT license and BLS certification.
Effective communication skills, customer service orientation, teamwork, and basic computer proficiency.

Responsibilities

Implement and revise treatment plans collaboratively with patients and families, providing education, discharge instructions, and referrals.
Evaluate patients’ needs, establish treatment goals, and develop individualized plans of care in coordination with physicians and families.
Accurately document evaluations, treatment sessions, and discharge plans according to departmental guidelines.
Maintain ethical standards, confidentiality, punctuality, and professionalism while fostering teamwork and effective communication.
Deliver exceptional customer service by applying AIDET principles, promoting a positive environment, and supporting patient and staff needs.
Engage in professional development, mandatory training, and participate in orientation and training of new staff.
Adhere to organizational policies, safety protocols, infection prevention, and participate in process improvement activities.
