Job summary

Rehabilitation Hospital of Oak Creek, part of Lifepoint Health, Inc., seeks a Licensed Case Manager to coordinate the interdisciplinary care plan, discharge planning, and patient progress communication within an inpatient rehabilitation setting. You will serve as a liaison among patients, families, and the healthcare team to ensure timely, effective transitions.

Responsibilities include coordinating weekly team conferences, reviewing CMG findings with HIM, and supporting seamless care

Qualifications

• Licensure as RN, Social Worker, Respiratory Therapist, Physical Therapist, Occupational Therapist, or Speech-Language Pathologist.
• Minimum 2 years inpatient social work or case management experience.
• Case management or rehab nursing certification preferred (CCM/ACM/RN-BC/ARN).
• Proficiency in Microsoft Office and strong English communication.

Responsibilities

• Coordinate the interdisciplinary care plan and IPoC per CMS guidelines.
• Lead discharge planning and ensure timely transitions.
• Schedule family conferences and communicate progress with caregivers.
• Arrange weekly interdisciplinary team conferences to monitor goals.
• Review CMG and coordinate with HIM on active comorbid conditions.
