Ocean Healthcare in New Jersey seeks a Social Worker (Psychiatric Hospital) for part-time, in-person shifts in the Acute Inpatient Behavioral Health unit. The role focuses on psychosocial assessment, discharge planning, and family engagement as part of an interdisciplinary team.

Required: MSW from an accredited program and an active NJ social work license (LSW or LCSW). New graduates are welcome. The position emphasizes accurate documentation and collaborative care planning.

Formación

• MSW from an accredited program.
• Current New Jersey LSW or LCSW license; new graduates welcome.
• Behavioral health or psychiatric experience preferred.
• Ability to document accurately and within established timeframes.

Responsabilidades

• Complete psychosocial assessments for assigned patients per policy and standards.
• Develop and coordinate individualized discharge and disposition plans.
• Engage family members and community supports as appropriate.
• Provide family sessions and education related to hospitalization and discharge.
• Obtain collateral information from family, providers, and supports.
• Participate in rounds and meetings to communicate psychosocial findings and interventions.
• Collaborate with psychiatrists, nursing, and other disciplines to support care.
• Coordinate with outpatient providers and community agencies for post-discharge care.
• Prepare documentation for mental health court proceedings when required.
• Maintain accurate EHR documentation and complete required training.
