Qualifications

• Master's degree in Mental Health & Behavioral Medicine or Social Work required.
• Clinical experience in a medical setting (2-3 years) required.
• Licensure or certification preferred (LCSW/LICSW/LMHC) per HR policy.

Responsibilities

• Provides psychosocial assessments of patients and families with social, emotional, interpersonal and/or environmental issues.
• Formulates biopsychosocial assessment, disposition, and treatment plans.
• Collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs as part of the interdisciplinary team.
• Monitor, evaluate, and record client progress according to measurable goals described in the treatment and care plan.
