Job summary

DCH Health System operates the North Harbor acute inpatient psychiatric program on the Northport campus, serving adults and geriatric patients with 24-hour care and a multidisciplinary team approach. The role focuses on psychosocial assessments, discharge planning, and group-based therapy within a secure, patient-centered environment.

The MSW (or Counseling/Related Master’s) background and strong communication skills are essential to coordinate care with families and the treatment team.

Qualifications

• MSW in Social Work required; LCSW preferred.
• Counselor track requires Master’s Degree in Counseling, Psychology, or Marriage and Family Therapy with License or Certification as required by discipline.
• Two years of inpatient hospital experience desirable.
• Strong communication and problem solving skills.
• Must read, write, speak, and comprehend English.

Responsibilities

• Perform psychosocial assessments and provide comprehensive social work services to assigned patient caseload.
• Coordinate patient treatment plan to include patient/family education, discharge planning, placement and follow up.
• Conduct groups to meet active treatment requirements.
• Document psychosocial assessments and groups in patient’s medical record; complete assessments timely with family involvement if permitted.
• Participate in interdisciplinary treatment team to coordinate patient care needs; contribute to Master Treatment Plan.
• Perform admission and discharge CQI data on each assigned patient.
• Ensure safety, adhere to policies, and maintain productive work schedule.
