Job summary

UofL Health, Inc. seeks a licensed therapist to evaluate and treat patients at Frazier Rehab Institute - UMC in Louisville, KY. You will develop and implement treatment plans, deliver therapy services, and discharge planning as part of a collaborative care team.

Responsibilities include regular re-assessments, clinical documentation, and interpretation of treatment outcomes to optimize functional independence while aligning with organizational goals.

Qualifications

• BS, MS or Doctorate level degree from accredited college or University (required).
• Licensed or eligible in the Commonwealth of Kentucky or State of Indiana (required).
• Basic Life Support (BLS) accreditation by the AHA (required).
• Proficient in Microsoft Office and EMR/revenue cycle systems.

Responsibilities

• Evaluates and implements competent therapy treatment and provides regular re-assessments to maximize functional independence.
• Communicates patient status to physicians and therapy customers with timely written or verbal updates.
• Maintains effective working relations with patients, families, and staff.
• Oversees rehab technicians and students per policy and acts as a role model.
• Ensures documentation and patient charges comply with regulatory standards.
