Job summary

UofL Health is seeking a licensed therapist at Frazier Rehab Institute in Louisville, KY to evaluate patients, develop and implement treatment plans, and coordinate discharge planning as part of an interdisciplinary team.

You will deliver evidence-based therapies across rehab services, communicate with physicians, patients, and families, and participate in program development and performance improvement while maintaining compliance with regulatory standards.

Qualifications

• Education: BS, MS or Doctorate level degree from accredited college or University (required).
• Licensed or eligible in the Commonwealth of Kentucky or State of Indiana, as appropriate to the facility (required).
• Basic Life Support (BLS) certification by the AHA (required).
• Experience varies based on programmatic needs and facility requirements.

Responsibilities

• Evaluates and treats patients; develops and implements treatment plans.
• Communicates patient status and goals to physicians and therapy customers.
• Oversees staff and ensures compliance with policies and procedures.
• Participates in program development and performance improvement activities.
