Job summary

Qualifications

• Bachelor's degree in Physical Therapy from an accredited institution is required.
• Active Physical Therapist license in state of practice.
• Graduates may practice under GPT or temporary license with supervision.
• Active BLS certification.

Responsibilities

• Examine and evaluate patients.
• Establish diagnosis, prognosis, and individualized care plans.
• Implement treatment interventions according to plan of care.
• Direct and coordinate rehabilitation plan; monitor progress and revise goals.
• Educate patients and caregivers; document and justify services.
