Job summary

Diagnostic Laboratory Services, Inc. Hawaii location is seeking a Pharmacy Technician to support pre-admission medication reviews.

You will interview patients or caregivers to obtain complete medication lists, including name, dosage, frequency, route, and last dose, and ensure accurate entry into the inpatient EMR. We require Hawaii registration, six months of pharmacy tech experience or three months plus program completion, and knowledge of drug names, medical terminology, and accurate

Qualifications

• High School diploma or equivalent.
• Current Hawaii State certificate of registration as a Pharmacy Technician.
• Completion of pharmacy technician program preferred.
• Certification by the Pharmacy Technician Board (PTCB) preferred.

Responsibilities

• Conducts interviews with patient and/or caregivers to obtain a complete list of medications (name, dosage, frequency, route, last dose) prior to hospital admissions.
• Enters medication list into the inpatient electronic medical record.
• Escalates any discrepancies that need further clarification to the covering clinical pharmacist.
• Performs duties of a Pharmacy Technician as needed.
