Job summary

The Queen\'s Health Systems Corporation in Honolulu, HI seeks a Pharmacy Medication Recon Tech to join our Pharmacy team. This full-time position (40 hours per week, rotating shifts) focuses on compiling comprehensive medication lists through patient interviews and pharmacy records, ensuring accuracy before admission.

You will enter the medication list into the inpatient EMR, escalate discrepancies to the clinical pharmacist, and perform Pharmacy Technician duties as needed.

Qualifications

• High School diploma or equivalent.
• Hawaii State Pharmacy Technician registration is required.
• Completion of a pharmacy technician program is preferred.
• PTCB certification preferred.

Responsibilities

• Interview patient and/or caregivers to obtain the most complete medication list before admissions.
• Enter medication list into the inpatient EMR.
• Escalate discrepancies to the covering clinical pharmacist.
• Perform duties of a Pharmacy Technician as needed.
