Job summary

Kaiser Permanente in Moreno Valley, CA is seeking a Pharmacist to monitor and manage medication therapy for hospitalized and ambulatory patients under supervision. You will provide dosing guidance, review orders, and collaborate with physicians and nurses to optimize outcomes.

The role requires a state license, a PharmD, residency or equivalent inpatient experience, and ACLS/BLS within 3 months. Ten-hour shifts may vary in start times.

Qualifications

• Minimum 1200 hours of inpatient pharmacy experience within the last 5 years.
• Degree in Pharmacy meeting licensure requirements.
• Pharmacist License (California) required at hire and NPIs as applicable.
• ACLS and BLS certification within 3 months of hire from American Heart Association.
• Demonstrated knowledge of patient variables, pharmacotherapy, and formulary adherence.

Responsibilities

• Upholds Kaiser Permanente policies and procedures, and applicable laws.
• Performs sterile and non-sterile compounding.
• Collaborates with health care team to optimize medication therapy.
• Reviews orders for appropriateness and formulary adherence.
• Interprets data to assess and adjust therapy; prevents medication errors.
