Kaiser Permanente in San Marcos, CA is seeking a part-time Pharmacist to oversee inpatient medication therapy and collaborate with the health care team to ensure safe, effective pharmacotherapy.

You will review orders, provide dosing advice, counsel patients, and participate in rounds, quality programs, and medication safety initiatives in a busy hospital setting. CA pharmacist license and 1200 hours inpatient experience preferred.

Qualifikationen

• Minimum 1200 hours of inpatient pharmacy experience within the last 5 years or completion of an ASHP-accredited residency.
• Pharmacist licensure in California is required at hire.
• Knowledge of hospital pharmacy policies, formulary and safety standards.

Aufgaben

• Monitor and adjust medication therapy in collaboration with the health care team.
• Provide pharmacokinetic dosing advice to medical personnel.
• Counsel patients on medications and device usage when applicable.
• Review medication orders for appropriateness and formulary compliance.
