Qualifications

• Minimum 1200 hours inpatient pharmacy experience within the last 5 years (intern pharmacist or residency).
• Degree in Pharmacy meeting minimum education requirements for licensure.
• Controlled Substance Utilization Review and Evaluation System Registration required at hire.
• Pharmacist License (California) required at hire.
• National Provider Identifier required at hire.
• ACLS within 3 months of hire.
• BLS within 3 months of hire.

Responsibilities

• Monitor and manage medication therapy for hospitalized or ambulatory patients.
• Collaborate with health care team to optimize therapy and develop patient-specific plans.
• Advise on dosing, orders, and laboratory tests per protocol.
• Provide patient education on medication use, safety, and adherence.
• Perform sterile and non-sterile compounding and verify accuracy of dispensing.
