Job summary

Kaiser Permanente Woodland Hills Medical Center is seeking a pharmacist to monitor and manage medication therapy for hospitalized and ambulatory patients. You will collaborate with the care team, perform sterile and non-sterile compounding, and provide pharmacokinetic dosing advice.

The role requires inpatient pharmacy experience (or ASHP residency), a California pharmacist license, and ACS/CPR certifications.

Qualifications

• 1200 hours inpatient pharmacy experience within the last 5 years or completion of an ASHP-accredited residency.
• Degree in Pharmacy meeting minimum education requirements for licensure.

Responsibilities

• Upsholds Kaiser Permanente policies and procedures and applicable laws.
• Performs sterile and non-sterile compounding.
• Develops and implements patient-specific plans to optimize medication therapy.
• Monitors therapy to ensure optimum results.
• Orders and adjusts dosages and administers medications under protocol.
• Interacts with patients and families to obtain medication information and provide counseling.
• Documents information in patient records and communicates with health care team.
