Job summary

Duke University Hospital in Durham, NC, is seeking a pharmacist for Inpatient Pediatric Pharmacy to provide comprehensive pharmaceutical care across the patient care continuum. You will oversee dispensing, drug information, and documentation within the Duke Health System, ensuring compliance with laws and Joint Commission standards.

The role involves reviewing therapy, ensuring medication availability, supervising personnel, and contributing to education of students and residents.

Qualifications

• Doctor of Pharmacy Degree OR BS Pharmacy Degree.
• Licensed Pharmacist or eligible to practice pharmacy in North Carolina.
• PGY2 Pediatric Pharmacy residency or PGY1 Pharmacy Residency with pediatric focus plus clinical experience preferred.

Responsibilities

• Review and monitor patient's medication therapy for safety and efficacy; advise patient care team of issues.
• Prepare, label and dispense medications per physicians' prescriptions; maintain required records.
• Maintain dispensing records and oversee medication charging/credit systems.
• Educate caregivers, patients and students about medications and proper use.
• Oversee the medication process; collect data and present reports on medication systems.
• Ensure medications are available, procured, prepared and stored properly.
• Supervise staff involved in preparation and dispensing of drugs and supplies.
• Provide oversight for investigational medications and ensure proper documentation.
• Provide specialized pharmaceutical services to improve drug usage and outcomes; advise on dosing and toxicity.
• Contribute to education of students and pharmacy residents.
• Maintain liaison with medical and nursing staff and ensure compliance with regulations.
• Conduct medication histories, assess compliance, participate in rounds.
• Individualize regimens considering pharmacodynamics/pharmacokinetics.
• Conduct and participate in research impacting patient care and pharmacoeconomics.
• Perform other related duties incidental to the work.
