Job summary

NJ Palliative Care Solutions is hiring a Nurse Practitioner or Physician Assistant to provide hospital-based palliative care at Monmouth Medical Center for patients living with serious illness.

Primarily a Monday–Friday schedule with 28 PTO days, a $0/month employee-only medical option, and CME/tuition support. Base salary $130,000–$160,000/year plus annual performance bonus up to 12.5% and robust benefits.

Qualifications

• Active New Jersey Nurse Practitioner or Physician Assistant license and national board certification.
• Palliative care experience required for inpatient role.

Responsibilities

• Provide hospital-based palliative care consultations and follow-up visits across the hospital continuum.
• Lead goals-of-care and advance care planning conversations with patients and families.
• Manage complex symptoms for patients with advanced illness and chronic disease.
• Collaborate with interdisciplinary team to integrate palliative medicine into hospital workflows.
• Complete timely documentation aligned with clinical, billing, and quality standards.
• Support discharge planning and transitions to patient’s next setting.
