Qualifications

• BC Nurse Practitioner or Physician Assistant with clinical experience in palliative care or geriatrics.

Responsibilities

• Assess and manage symptoms for patients with advanced illness.
• Provide consults to hospital teams in a multidisciplinary model.
• Conduct family meetings and discuss goals of care with patients and families.
• Develop goal-concordant plans of care in collaboration with the care team.
