Job summary

Advocate Health's Wake Forest Baptist Medical Center Pharmacy is seeking a Clinical Pharmacist for Inpatient Hematology & Oncology. This full-time role focuses on adult and pediatric chemotherapy, with Monday–Friday scheduling and weekend rotation to be determined, located at Winston Campus, 10CC.

The pharmacist will oversee care, educate patients and caregivers, design and monitor regimens, ensure safety, and collaborate with the healthcare team.

Qualifications

• Bachelor’s or Doctorate in pharmacy from ACPE-accredited school or an FPGEC certificate.
• Completion of PGY1 preferred and PGY2 oncology preferred.
• Licensed in the state(s) of practice or eligible for reciprocity.
• Immunization and non-vaccine injectable certification required based on area of practice.
• Credentialing and privileging required based on area of practice.

Responsibilities

• Oversees pharmaceutical care and designs medication therapy regimens.
• Counsels patients and caregivers on therapy, adverse effects, and adherence.
• Designs and monitors patient-specific pharmacotherapeutic regimens.
• Implements recommendations and documents per policies and procedures.
• Reviews medication orders and participates in emergencies.
• Supports medication safety and quality improvement activities.
• Mentors pharmacy teammates and serves as a preceptor.
• Ensures proper storage, handling, and regulatory compliance.
