Responsibilities

• Performs and documents an initial examination to identify problems and plan treatment.
• Plans and carries out individually designed treatment plans to improve or restore function and alleviate pain.
• Re-evaluates treatment effectiveness and adjusts interventions for maximum benefit.
• Communicates with patients and medical practitioners regarding outcomes and expectations.
• Records prognosis, treatment, response and progress in EMR and provides discharge reports.
• Collaborates with physicians, allied health professionals, nursing and case managers and families.
• Participates in discharge planning to optimize long‑term outcomes.
• Engages in community activities to raise awareness of Winchester Hospital services.
• Transfers patient with appropriate assistive device to minimize risk.
• Responds to supply/equipment issues to ensure availability.
