Job summary

Jefferson Health is seeking an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to join the inpatient neurosurgery service within an acute care hospital setting. The role involves close collaboration with a multidisciplinary health‑care team to deliver comprehensive care during admission, procedures, and transitions.

The position emphasizes performing histories and exams, ordering and interpreting tests, and delivering evidence‑based, individualized plan‑of‑care.

Qualifications

• Master’s degree.
• National Board Certification as an Acute Care Nurse Practitioner or Physician Assistant.
• BLS certification.

Responsibilities

• Perform complete history and physicals and document in the medical record.
• Order and collect data using appropriate assessment techniques and relevant diagnostic information.
• Serve as first responder for patient status changes, orders, and procedures requiring resuscitative interventions.
• Assess risks associated with acute and chronically ill patients, including medication side effects, immobility, nutrition, fluid and electrolyte imbalance, immunocompetence, invasive interventions, and diagnostic procedures.
• Collaborate with interdisciplinary health‑care team to diagnose acute and complex chronic conditions.
• Manage diagnostic tests through ordering, interpretation, performance, and supervision.
• Formulate differential diagnoses by priority.
• Diagnose complications and order appropriate interventions.
• Identify expected outcomes and formulate a plan of care to address complex acute and chronic health needs.
• Utilize evidence‑based practice guidelines and protocols in an individualized, dynamic plan of care applicable across the continuum.
• Implement and modify plan of care; prescribe diagnostic strategies and therapeutic interventions.
• May perform advanced procedures consistent with privileges and competency validation.
• Maintain ongoing communication and collaboration with interdisciplinary team.
• Facilitate and communicate with patient, family, and staff to promote continuity of care across inpatient and outpatient settings.
• Document key components of patient progress via daily progress notes, transfer, discharge summary, and clinic notes where applicable (H&P, problem lists, procedure notes, acute event notes, death summary).
• Ensure documentation is timely, meets acute care compliance standards, and captures patient acuity.
• Demonstrate professional practice behaviors including preceptor/mentoring, education, and instruction of graduate students.
• Seek opportunities for active engagement in research and evidence‑based practice analysis.
• Participate in Advanced Practice Grand Rounds and specialty‑specific meetings.
• Maintain CEUs and membership in a professional organization.
