Job summary

Providence Little Company of Mary Torrance seeks a Care Manager MSW to provide patient-centric care management in an acute care setting. This full-time, day-shift role emphasizes assessment, planning, coordination of care, discharge planning, and advocacy to ensure continuity of care for at-risk patients.

The MSW must hold a CSWE-accredited master’s in social work with at least one year of healthcare experience, preferably with case management exposure.

Qualifications

• Master's Degree in Social Work from an accredited CSWE program.
• 1 year Social Work experience in any healthcare setting.
• 1 year Experience in acute care setting or TIPs/Case Management Orientation program.

Responsibilities

• Assess and plan to identify patient needs and care gaps.
• Coordinate care and arrange discharge planning to ensure continuity of care.
• Document interventions and monitor compliance with regulatory requirements.
• Advocate for patients to address social, financial, cognitive or legal barriers to access.
