Qualifications

• Master's Degree in Social Work from an accredited CSWE program.
• Texas Masters Social Worker License required upon hire.
• 1 year of social work experience in a healthcare setting.
• 1 year of experience in an acute care setting or completion of TIPs/Case Management Orientation program.

Responsibilities

• Provide assessment and planning, coordination of care, and discharge planning.
• Document interventions and ensure regulatory compliance.
• Advocate for patients and address social, financial, cognitive/behavioral or legal barriers to care access.
