Job summary

Thomas Jefferson University Hospitals, Inc. is seeking an Inpatient Care Manager with an MSW to coordinate comprehensive patient care across the continuum.

The role includes assessing, planning, implementing, and evaluating care plans while ensuring timely discharge planning and collaboration with the interdisciplinary team. The position requires leadership, experience in care coordination, and knowledge of Medicare/Medicaid rules.

Qualifications

• MSW degree required.
• Preference for LCSW/CCM licensure or eligibility within 3 years.
• Experience with discharge planning and care coordination is expected.

Responsibilities

• Assess and plan individualized care and discharge plans with the multidisciplinary team.
• Coordinate safe discharge, post-acute arrangements, and transfers as needed.
• Monitor patient progress and participate in rounds to remove barriers to discharge.
• Advocate for patients and families, addressing clinical, educational, and psychosocial needs.
• Coordinate team and family conferences to progress care.
• Refer to community-based services to meet continuum of care needs.
• Maintain Medicare/Medicaid knowledge and reimbursement methods.
