Job summary

Jefferson Health seeks an Inpatient Care Manager-MSW to coordinate comprehensive care across the continuum for a designated patient population. You will assess, plan, implement, and evaluate individualized care plans while ensuring safe discharge planning and efficient transitions.

You will collaborate with an interdisciplinary team, advocate for patients, and manage post‑acute authorizations, leveraging EPIC and Medicare/Medicaid knowledge to optimize outcomes.

Qualifications

• Master’s Degree in Social Work (MSW) required.
• Less than 1 year MSW internship experience preferred.
• Ability to interact with diverse backgrounds.

Responsibilities

• Conducts comprehensive assessments at entry to determine LOS and discharge needs.
• Coordinates safe, timely discharge planning including post-acute needs.
• Monitors patient progress and facilitates multidisciplinary rounds.
• Advocates for patients/families and reflects preferences in care plans.
• Collaborates with teams to develop and implement care plans.
• Provides referrals to community resources for continuum of care.
• Maintains knowledge of Medicare/Medicaid benefits and reimbursement.
