Qualifications

• Master’s degree in Social Work required.
• Less than 1 year of experience in an MSW internship program.
• Ability to interact with persons from diverse backgrounds.
• Knowledge of computer applications and analytical tools.
• Proven leadership skills.

Responsibilities

• Conducts comprehensive assessments at patient entry to determine length of stay and discharge needs.
• Develops and implements individualized plans of care and transition plans with the team.
• Coordinates safe and timely discharge planning, including post-acute authorizations and transportation.
• Monitors patient progress and resolves barriers to discharge.
• Educates patients and families to support self-management and adherence to care plans.
