Virginia Commonwealth University Health Systems seeks an Inpatient Social Worker (MSW) to support care coordination and transitions of care. The role requires strong clinical judgment and collaboration with interdisciplinary teams to ensure safe, patient-centered discharge planning and community resource navigation.

The position emphasizes direct patient/family care, psychosocial assessments, treatment planning, and education.

Competenze

• Master of Social Work degree from a CSWE-accredited program.
• Licensed Clinical Social Worker in Virginia preferred.
• CSW Level 2 with at least 1 year of clinical social work experience.
• Clinical social work experience in an acute care setting preferred.

Mansioni

• Complete psychosocial assessment and develop aligned treatment plans.
• Provide psychosocial intervention and education to patients, families, staff, and social work students.
• Coordinate discharge planning and community referrals to support transitions of care.
• Report suspected abuse/neglect/exploitation to the appropriate Social Services Department.
• Develop intervention plans with patients, families, and significant others.
• Educate facility staff and participate in quality improvement initiatives.
• Participate in service area meetings, trainings, and patient care conferences.
