Job summary

VCU Health System Authority is seeking an Inpatient Social Worker (MSW) to join our Care Coordination team in Richmond, VA. You will assess psychosocial needs, develop treatment plans, and coordinate discharge planning for patients and families within an interdisciplinary care framework.

The role requires a Master of Social Work from a CSWE-accredited program, and preference for Virginia licensure as a clinical social worker.

Qualifications

• Master of Social Work Degree from CSWE-accredited program.
• Virginia licensure as clinical social worker preferred.
• Minimum 1 year of clinical social work experience (CSW Level 2).
• Acute care experience preferred.

Responsibilities

• Perform psychosocial assessments and develop aligned treatment plans.
• Provide psychosocial interventions to patients and families.
• Educate patients, families, and staff on care planning.
• Coordinate discharge planning and community referrals.
• Collaborate with interdisciplinary teams for transitions of care.
• Report suspected abuse/neglect to social services as required.
• Contribute to quality improvement through service analysis.
• Educate and mentor social work students and staff.
