Collaborate with physicians, nurses, rehab and case management to facilitate transitions to appropriate care levels and optimize outcomes.

Qualifications

• Master's degree in Social Work is required.
• Experience with computer systems including web-based applications and Microsoft Office (Outlook, Word, Excel, PowerPoint, OneDrive, SharePoint).

Responsibilities

• Provides clinical social work services to patients and families to support emotional, social, and physical well-being and appropriate use of health care resources.
• Screens and triages patients requesting services or identified for social work evaluation.
• Conducts psychosocial assessments and provides services as indicated.
• Collaborates with multidisciplinary team to facilitate continuity of care planning.
• Meets productivity standards set by Social Work leadership and assigned area.
• Provides counseling and psychotherapeutic services appropriate to setting.
• Documents communication and care in the medical record in a timely manner.
• Offers education on psychosocial issues affecting patient care and ethical considerations.
