Job summary

CHOT is seeking a Licensed Vocational Nurse to join our Hospice team in Burleson, TX. You will assess patients’ physical, psychosocial, and spiritual needs under supervision and contribute to the plan of care, providing compassionate palliative nursing.

You will assist the RN with case management, coordinate with the interdisciplinary group, document nursing actions, support families, participate in admissions and discharges, manage crises, and assist with pharmacology as required.

Qualifications

• Licensed Vocational Nurse licensed by the State of Texas.
• 1 year of nursing experience is required.
• Hospice/home health experience preferred.
• Proficient computer skills; Microsoft Office experience required.

Responsibilities

• Assess the patient's physical, psycho-social, and spiritual condition; contribute to the plan of care in collaboration with an RN and provide palliative nursing care.
• Provide emotional support for patients and families/caregivers while instructing in end-of-life care/issues.
• Assist RN in case management of total plan of care of patient/support persons by coordinating with team members, communicating with IDG, and supervising CNAs.
• Document nursing care and significant interactions with patient support persons and team members.
• Participate in follow-up with bereaved families with various types of contact.
• Assist the RN in the admission of a patient to Hospice House, including immediate crisis management and orientation.
• Assist RN in the discharge of patients and coordination of transfer, including instructing patients/caregivers in follow-up care.
• Assist the RN in the management of multiple patients and their support persons simultaneously in crisis with acute symptoms, including pharmacology.
