Job summary

Uvalde Memorial Hospital’s Hospice team is seeking an LVN to provide compassionate palliative nursing in patients' homes across the service area. This PRN role requires travel, on-call participation, and strong collaboration with an interdisciplinary team to maintain comfort and dignity at end-of-life.

Qualified candidates hold a Texas LVN license, BLS certification, and have good communication, organization, and empathy.

Qualifications

  • Current LVN license valid in Texas.
  • BLS for Healthcare Providers certification required.
  • One year of LVN experience preferred, hospice experience preferred.
  • Compassionate and professional communication with patients and families.
  • Strong organization and ability to manage multiple priorities.
  • Proficient with Microsoft Office, Outlook and electronic documentation.

Responsibilities

  • Provide compassionate palliative nursing care in patients' homes per plan of care.
  • Monitor patient conditions and assist with pain and symptom management.
  • Report changes to supervising RN promptly.
  • Support physical, emotional and psychosocial needs of patients and families.
  • Communicate with RN Case Managers after visits to ensure continuity of care.
  • Collaborate with physicians and the hospice team.
  • Maintain accurate, thorough patient care documentation.
  • Participate in interdisciplinary meetings and education.
  • Support a safe, respectful environment for patients and families.
  • Participate in on-call responsibilities as assigned.

In This Role, You Will:
  • Provide compassionate palliative nursing care in patients’ homes according to the established plan of care and within the LVN scope of practice.
  • Monitor patients’ conditions and assist with pain and symptom management.
  • Recognize and promptly report changes in a patient’s condition or response to treatment to the supervising RN.
  • Provide care that supports the physical, emotional, and psychosocial needs of patients and their families.
  • Communicate with RN Case Managers following patient visits to support continuity of care.
  • Collaborate with physicians, nurses, and other members of the interdisciplinary hospice team.
  • Maintain accurate, thorough, and timely patient care documentation.
  • Participate in interdisciplinary meetings, in-service education, and performance improvement activities.
  • Help create a safe, respectful, and supportive environment for patients and families.
  • Participate in on-call responsibilities as assigned.
What We’re Looking For:
  • CurrentLicensed Vocational Nurse (LVN) license in the State of Texas required.
  • Current BLS for Healthcare Providers certification required.
  • Valid driver’s license, current automobile insurance, and reliable transportation required.
  • One year of LVN experience preferred.
  • Previous hospice experience preferred.
  • Compassionate and professional communication skills with patients, families, and members of the healthcare team.
  • Strong organizational skills with the ability to manage multiple priorities.
  • Proficiency with Microsoft Office, Outlook, and electronic documentation.
  • A dependable, patient-centered professional who understands the importance of comfort, dignity, and compassionate end-of-life care.

This position includeshome visits and travel throughout the hospice service area. Work hours may include nights, weekends, and holidays, and participation in the on-call rotation is required.
