Job summary

Community-Healthcare-of-Texas seeks a PRN Licensed Vocational Nurse to provide compassionate hospice nursing care in Burleson, TX. You will work with an RN, assist in admissions and discharges, and support families coping with end-of-life issues.

This role requires TX LVN licensure and at least 1 year of nursing experience; hospice/home health experience is preferred. Proficiency with computers and Microsoft Office is expected to document care and coordinate with the care team.

• Assess the patient's physical, psycho-social, and spiritual condition; contribute to the plan of care in collaboration with an RN and provide palliative nursing care.
• Provide emotional support for patients and families/caregivers while instructing in end-of-life care/issues.
• Assists RN in case management of total plan of care of patient/support persons by coordinating with team members, communicating with IDG, and supervising CNAs.
• Document nursing care and significant interactions with patient support persons and team members.
• Participate in follow-up with bereaved families with various types of contact.
• Assist the RN in the admission of a patient to Hospice House, including immediate crisis management and orientation.
• Assist RN in the discharge of patients and coordination of transfer, including instructing patients/caregivers in follow-up care.
• Assist the RN in the management of multiple patients and their support persons simultaneously in crisis with acute symptoms, including pharmacology.

POSITION QUALIFICATIONS

• Licensed Vocational Nurse licensed by the State of Texas required
• 1 year of nursing experience is required
• Hospice/home health experience preferred
• Proficient skill using a computer is required; Microsoft office suite experience required
