Job summary

Carle Health is seeking an experienced Licensed Practical Nurse (LPN) to provide direct nursing care under the supervision of a registered nurse in our Illinois facilities.

Responsibilities include administering medications, including IV piggybacks, performing wound and trach care, NG feeding, documenting assessments, and coordinating with RNs and physicians to ensure safe discharge planning.

Qualifications

• Certification: Licensed Practical Nurse (IDFPR)
• Basic Life Support (BLS) within 30 days (AHA)
• Education: College Diploma: Licensed Practical Nurse (Required)
• Certificate of Completion: Licensed Practical Nurse

Responsibilities

• Provide direct nursing care to patients under the supervision of a registered nurse.
• Administers medications, including IV piggybacks and fluids but excluding IV push medications.
• Documents assessments on assigned patients, once RN has charted initial assessment.
• Completes dressing changes and wound care.
• Completes trach care.
• Inserts NG tubes and maintains feedings through NG tube.
• Cares for and changes ostomies.
• Reinforces education and discharge instructions.
• Reports patient response of care to a registered nurse for evaluation, intervention, and modification of the plan of care.
• Coordinates care with physicians/APPs to receive orders.
• Takes responsibility for patient and employee safety by reporting hazards.
