Job summary

Carle Health in Illinois is seeking a Licensed Practical Nurse (LPN) to provide direct patient care under RN supervision. The role supports the nursing care plan and participates in patient education and documentation.

Responsibilities include administering medications, assisting with procedures, and coordinating care with physicians. The LPN will also participate in triage and ensure accurate charting within the EMR.

Qualifications

• LPN certification (Illinois)
• BLS certification within 30 days
• Education: College Diploma for LPN

Responsibilities

• Provide direct nursing care to patients under supervision of a nurse, midlevel provider or physician.
• Report patient responses to care for evaluation and modification of the care plan.
• Administer medications via oral, rectal, IM and inhalation routes; assist with procedures.
• Be a member of the Code Team when needed; preceptor for new staff; clean instruments.
• Room patients and collect pertinent information, vitals, and medication reconciliation.
• Handle telephone triage and document actions in the EMR; communicate test results to patients.
• Perform office procedures as directed, including sutures, splints, catheterizations, IV hydration, and TB test readings.
• Provide written follow-up instructions and assist with prescription refills per protocol.
