Job summary

Hospice of Acadiana, Inc. seeks an Inpatient Unit RN to provide skilled nursing assessment, planning, and care within the IPU. You will work as part of the IDT to maximize patient comfort and health in accordance with state nursing standards.

The role requires an active Louisiana RN license, valid BLS, and hospice experience preferred. You will participate in care planning, documentation, and education for patients and families, with day and night shifts in Lafayette, LA.

Qualifications

• Valid Louisiana RN License.
• BLS CPR Certification.
• CHPN (Hospice & Palliative Care Certification) preferred.
• Bachelor’s degree in nursing from a regionally accredited institution.

Responsibilities

• Perform initial and ongoing patient and family assessments to determine hospice needs.
• Develop and implement care plans with goals based on nursing diagnoses.
• Administer medications and treatments as prescribed; provide patient and family education.
• Record clinical notes and communicate changes with physicians and the IPU team.
• Coordinate patient care as Case Manager when assigned and participate in interdisciplinary team meetings.
